Jaw clicking and pain: understanding TMD

Understand temporomandibular disorders, why painless clicking may not need treatment and why conservative assessment comes first.

The temporomandibular joints connect the jaw to the skull. Temporomandibular disorders, or TMDs, affect these joints or the muscles used for chewing. Symptoms can include pain, stiffness and restricted movement.

Clicking without pain is common and does not automatically require treatment. Pain around the face can have several causes, so an examination is needed before attributing it to a jaw joint problem.

Many patients benefit from an initial conservative approach. A clinician may discuss habits such as clenching, short-term dietary adjustments and other measures suited to the diagnosis. Do not begin permanent bite changes or surgery simply because a jaw clicks.

Ask what diagnosis is being considered, which alternatives exist and when specialist review is needed. Seek assessment for painful locking, difficulty opening or persistent symptoms. Keep a note of triggers and the effect on eating to help the clinician understand the problem.
