Persistent pain after root canal treatment

Understanding why some discomfort may remain after a root canal and when to seek further advice from a dentist.

It is normal to experience some mild tenderness for a few days following root canal treatment as the surrounding tissues heal. However, if you experience persistent or worsening pain after the initial recovery period, it is important to investigate the cause. While the internal nerve of the tooth has been removed, the ligaments and bone around the tooth are still very much alive and can become inflamed.

One reason for lingering pain could be a hidden canal that was not fully cleaned during the initial procedure. Teeth sometimes have very complex anatomy with extra channels that are difficult to see. Another possibility is a microscopic crack in the root or a reinfection caused by a leaking temporary filling. If the bite on the new restoration is slightly too high, this can also cause significant discomfort when chewing.

In some cases, the body takes longer to resolve a pre-existing infection in the bone. If you had a large abscess before the treatment, the healing process in the jawbone can take several weeks. However, if you notice severe throbbing, visible swelling, or a fever, you should contact your dentist immediately, as these may be signs that the infection is still active and requires further attention.
